At the base of this project: 3 doctors trained at the APHP who appreciated the permanent exchange with the various general practitioners and specialists and who wanted to offer patients complete care and continuity of care.

Courses have been drawn up for each pathology and will be applied by all the practitioners in our structures.
Because benevolence, warmth, right to care are the watchwords of Cape Horn. The professionals in our clinics make every effort to guarantee continuity of care.

La collaboration entre les différents intervenants de notre maison de santé grâce au dossier médical partagé et aux réunions de concertation pluridisciplinaires mensuelles garantit aux patients une prise en charge coordonnée et sérieuse au sein de notre structure.
Nous avons également établi des collaborations avec plusieurs structures hospitalières qui se sont engagées à prendre en charge nos patients rapidement si nécessaire.
